Wilson Health and Orthopedic Associates are pleased to announce that they have scheduled three days for sports physicals.
The cost is $15 and is open to the community. Appointments are required. The dates and locations are:
Wednesday, July 1 from 4 pm – 8 pm at the Wilson Health Medical Group Pediatric Office, Professional Building, Suite 200; Sidney. Call (937) 498-4880 to schedule an appointment.
Saturday, July 25 from 8 am – Noon at the Wilson Health Medical Group Pediatric Office, Professional Building, Suite 200; Sidney. Call (937) 498-4880 to schedule an appointment.
Wednesday, July 29 from 4 pm – 8 pm at the Orthopedic Associates Office, 300 Third Avenue; Sidney. Call (937) 710-9257 to schedule an appointment.
Pre-participation physical forms must be completed prior to appointment. Please limit appointments to only one child and one adult.
